How To Order De-Nol, Furazolidone, Nitazoxanide, Tetracycline and Dicylomine.HCL
There are two forms that you will need to fill out. (Only GP's and Specialists or their representatives can fill out these forms)
These blank forms are listed below on the website as:
TGA Category B Form.
Tri-Med Order and Payment Form
- The TGA's Category B
"Special Access Scheme" (SAS) Proforma Number 2950.
Print this out and fill in the details and then fax it to the TGA on fax number (02) 6232 8112.
If you should wish to ring them their number is (02) 623 28111.
NB: You will need to fill out a separate TGA "Category B form for each drug.

The doctor intending to prescribe will receive a letter by post from the Department of Health & Aging granting him/her authority to prescribe.
Please Note: The turn around time for this is 2 to 5 days plus postal service delivery time.
- The Tri-Med Order form.
You will need to print this out and fill this it in noting in particular the address the drugs are to be sent to with phone numbers and Payment details. We accept Visa and Master card or a patient can send a cheque money order with the TGA approval letter and Tri-Med order form to:

- Once the "Typed Permission Letter" is received from the TGA, fax this with the Tri-Med Order and Payment form to

- The medications will be sent by Express Post the same day they are received. This while normally an over night service can take up to 5 days.
