Arab Health Conference 2021: Helicobacter pylori – the Evolution of Treatment Protocols

Dr. Warren and Professor Marshall’s discovery of Helicobacter pylori and its link to peptic ulcers won them the Nobel Prize in 2005.

Once discovered, treatment of H. pylori in the stomach was found to only be achieved through precision antibiotic therapy.

The ‘Marshall Plan’ for treatment of H. pylori is as follows.

Step 1. Choose a base therapy that is not resistance inducing.

This is done using one or two of the following:

- Proton pump inhibitor (PPI) or any strong acid blocker

- Strong bismuth dose

- Amoxicillin

- Tetracycline

In order for treatment to be effective it was found that a PPI or a strong acid blocker was essential for a high eradiation rate to be achieved.

Step 2. Choose a ‘CIDAL’ antibiotic (or two)

- Clarithromycin

- Metronidazole

- Levofloxacin

- Rifabutin

 Do not repeat failed antibiotic treatments.

 Step 3. Always check for treatment success

Tests that can assess H. pylori presence:

1. C14-UBT

- Safe, simple and quick

- Tri-Med PYtest

- Tri-Med CLOtest

2. C13-UBT

- Safe, complicated and slower than C14-UBT
